1) All amounts not paid as agreed herein shall accrue interest at the rate of 1.5% per month (18% per annum) or the highest rate permissible.
2) Applicant agrees to pay a service charge of $25 for each check returned unpaid.
3) In the event of default in payment of any amount due hereunder, and/or if the account is placed for collection, Applicant agrees to pay all
collection costs, attorney fees and court costs.
4) No returns will be accepted without prior authorization and a "Return Authorization" having been issued by Equipment de Sport, USA
Incorporated. .
The above information is for the purpose of obtaining credit from Equipment de Sport, USA Incorporated and is warranted to be true and correct. I
hereby authorize Equipment de Sport, USA Incorporated to investigate the above references and to obtain credit information from them. I agree to
the terms and conditions of sale as stated above and on the order form itself. Should credit be granted, all decisions with respect to the
extension, continuation, or termination of such credit shall be in the sole discretion of Equipment de Sport, USA Incorporated.
